The Summer Foundation has partnered with Housing Hub to deliver a free specialist service, working directly with young people living in aged care, and their supporters, to achieve their housing and support goals.

Whether you want to move out of aged care or improve your quality of life while in aged care, we can support you to achieve your goals. The Aged Care to Home service supports the Federal Government’s target to ensure all younger people leave aged care by 2025 (apart from in exceptional circumstances).

There is no charge for the Aged Care to Home service, which is philanthropically funded.
